If you are a brand new patient to Dr. Cleaveland, please book the appropriate new patient exam; if she has seen you in the last year, please book as a returning patient. Regardless of new or returning patient status, if you are new to Jane App, you will be required to fill out new patient paperwork before being seen. As a reminder, Cleaveland Chiro LLC is in-network with: BCBS, Aetna, UHC, Harvard Pilgrim HC and MGB at this time; cash fees are listed on our booking portal. Thank you!
